New Patient Forms

If you need to contact your insurance carrier, please inform them of the specific therapist you will be seeing. All therapists are listed individually as participating providers for the following insurance companies:

- AETNA
- Blue Cross/Blue Shield
- Cigna *Not currently accepting new patients
- CBA
- FirstHealth
- Medicare

- Medicaid *Not currently accepting new patients
- MVP
- Tricare
- VA *Not currently accepting new patients

**Please keep in mind that all therapists are listed individually as participating providers with insurance companies. While we encourage all patients to call their Member Benefits office to verify participation, please be sure to know the name of the therapist you will be seeing to confirm eligibility.
